On-Demand Training

Given demographic trends in the United States, health centers are likely to care for an increasing number of Medicare-enrolled and Medicare-eligible patients. For enrollees, Medicare annual wellness visits (AWVs) can provide an opportunity to manage multiple chronic conditions and are increasingly required by payors. Therefore, the AWV and how it is operationalized are strategically important to Community Health Centers. The presenter is a practicing nurse at a Community Health Center in another state who now completes nurse-led AWVs as her full-time role. Topics to be covered include: -How the role evolved and what prompted this evolution -Financial implications—how nurse-led AWVs generate a positive ROI -How to effectively utilize team-based care in the context of AWVs -Scheduling, infrastructure, and other considerations.
